Every asthmatic patient
at the time of consultation to a doctor asks a common question
about what he should eat and what he should not eat. It is a
common belief that food allergy is one of the most important
precipitating factor in majority of patients. But this is not
true. Only about 5% patients of asthma have food allergies
and they can be benefited by avoidance of allergic foods.
However, in many asthmatic patients food can aggravate asthma
through other mechanisms. With advancing food technology,
preservatives and coloring agents are used much more
frequently.

aggravates asthma through, |

| Preservatives
are used to preserve food for longer durations while coloring
agents are used to make edibles more attractive. In sensitive
individuals these can aggravate asthma. Sulfites and MSG
treated foods such as sauce, pickles, treated grapes, wine,
apple, apricots and nuts can aggravate asthma in sensitive
individuals. Coloring agents are used in squash, toffees and
sweets. Such edibles can cause asthma. Usually in such
patients aspirin and other pain relieving medicines can also
induce asthma.

In
a great majority of asthmatic patients nocturnal worsening is
common. One of the most important explanation for this
phenomenon is acidity or gastro-esophageal reflux (GER). In
reflux, food contents of stomach are pushed in to esophagus.
Asthmatic patients with reflux have symptoms such as acidity,
bleaching and heart burn. Reflux is more common in
lying/recumbent posture and specially after heavy meals. Food
with higher contents of fat and proteins induces reflux more
frequently. If an asthmatic has nocturnal asthma with habit of
taking heavy meals, reflux should be considered as a
contributing factor.
Cold edibles such as ice-cream, sour drinks and oily edibles
are also considered as a cause of sore throat and aggravation of asthma by many patients.
Till a decade ago the role of food was considered only
significant for aggravation of asthma. However, recently some
foods are identified with asthma protection. Vitamin C, fish
intake, Zinc etc. are included in this category.

During
food processing sulfites, mono sodium glutamate (MSG), Azo dyes such

Usually they are the patients in whom asthma is aggravated by aspirin
and other pain relieving medicines. These include:
- Jam and Jelly
- Sauce
- Chocolate
- Butter
- Tinned Juice
- Processed fruits such as apple,
apricot, resin
- Wine
- Soft drinks, ice-creams, sweets.

Clues
of foods allergy
- Itching
in eyes or nose, nasal blockade, rhinorrhoea, urticaria or asthma starts
after taking allergic food.
- A food
positive reaction in allergy skin testing should be suspected as trigger
factor. If consumption of these suspected edibles is followed by an attack
of asthma then it confirms the food trigger factor.
- Avoidance
of the suspected food improves asthma.

Sweets cause asthma?
Example:
Six years old Tony had asthma for last 3 years. Bad
throat and house dust mite were identified as
causative factors of his asthma. His asthma was
deteriorating for last 6 months which led to increased
consumption of medicines.
Curiosity:
One day Tony's aunt came to
his house. She saw Tony taking toffees. She remembered
she had asthma
and it aggravated on taking colored sweets. On her
suggestion Tony stopped taking toffees. Surprisingly
his asthma improved.
Expert
Comments:
This was happening due to coloring
agent used in toffees.

| Preservatives
and coloring agents |
- Many
aspirin sensitive patients may also react to
food additives, preservatives and coloring
agent such as Mono sodium Glutamate (MSG),
Tartrazine, Benzoate and Sulphites. Therefore,
the aspirin sensitive patients should also be
cautious whilst consuming processed colored
food such as catch-up, jelly, chocolate, soft
drinks, ice-cream, cake and bakery goods
except plain bread, chewing gums, toothpaste,
perfumes and dry fruits.

| Ice-cream |
- It
should be taken very slowly . Half melted
ice-cream is better because it delivers less
cold due to loss of latent heat.
- Take
some sugar crystals after ice-cream.

| Sour
drinks, Butter Milk, Lemon water |
- Addition
of sugar in these edibles is found to be
useful by some patients.
- Alternatively,
after
taking these few sugar crystals may be
taken.

edibles was found useful by some patients.

a dose of inhaler before meals.

Dust
and Dinner induce Asthma?
Example:
Mr. Ram Narayan Choudhary, has been suffering from
asthma for last 5 years. He works at Food Corporation of
India. At the godown of FCI, he used to broom to collect
food grains spoiled all over. He felt a slight
irritation in throat and chest tightness. By evening he
had difficulty in breathing. When consulted, a doctor
diagnosed his disease as Bronchial Asthma. Since then
he has been suffering from this disease quite often. His
asthma gets aggravated after common cold or grain dust
exposure specially during night for months together. In
day time, Ram Narayan worked normally but during night
after taking meals his asthma problem aggravated. Even
medicines could not control his asthma properly.
Curiosity:
One day
just by chance, he had not take dinner and he felt much
better. Then he stopped taking dinner and took lunch
only. It worked quite well & now problem of his nocturnal
asthma is much better for the last one year.
Expert Comments:
The house
dust mite grows in damp places and the grains depots are
suitable sites of their growth. If a person is exposed
to a very high concentration of dust mite it can cause
asthma. Probably it happened with Ram Narayan. Once it
started Asthma problem persisted for long time, or may
be forever. Among non allergic trigger factors of
asthma, gastric-acidity or gastro-esophageal reflux
(GER) is an important aggravating factor. GER becomes
more intense on full stomach and on lying down.
Probably, it was responsible for nocturnal asthma of Ram
Narayan and this problem was solved as Ram Narayan
stopped taking dinner.

how Fume and Onion cause Asthma?
Example:
Ashok
Kumar, 25 years aged, was young cheerful man who
never fell ill in his young life. Bathing in rain,
riding scooter without wooll en clothing in chilled night
of December, or taking ½ Kg ice-cream or raw onions
never caused any problem to him. He was working in a
factory. On one day he had slight cold and went to kiln
site of factory. Since other workers had cough due to
intense fumes he stayed and worked in that
environment for two hours. At the end he had a strange
sense of chest tightness. But he never paid attention
and went home. In the night around 10 O'clock he got up
due to distress, as if some one was strangulating him.
He was profusely sweating and fighting for gasping air.
His brother took him to nearby doctor who diagnosed him
having bronchial asthma. Since then there were so many don'ts
that pleasure of free life was a matter of memories
only. Now for the last 3 years he is having asthma on
and off. Six months back he had severe asthma lasting for
two months. Medication intake had to be doubled but
still his symptoms could not be controlled.
Curiosity:
On keen
observation Ashok found that on the day of avoiding raw onion
he improved considerably and medicine were also reduced
to minimum. Even now, he gets asthma on the day of
taking raw onion but cooked onion never caused any
problem.
Expert Comments:
It has been
seen that sometimes occupational asthma starts after
exposure to industrial fumes.
Fumes makes airways more
reactive. Once started asthmatic tendency persists
indefinitely and other substances also trigger asthma.
It happened with Ashok Kumar. Onion by its nature of
strong irritant aggravated asthma. As soon as Ashok
Kumar stopped taking onion his asthma improved.

Soft
drink causes Asthma?
Example:
Kishan Lal had mild asthma since childhood. It used
to give him trouble 2-3 times in a year. However, for
the last 1 year severity of asthma was aggravated. He
consulted the best doctors of the city and took
medicines. Though he became symptom free, he was on high dose of
inhaled steroids. He and his doctor were trying hard
to find the reasons. Only peculiar change was that one year
back, when Kishan lal had huge profit in
business he
changed his old car, purchased a new house and started taking modern food. He stopped taking water
and instead took squash and other soft drinks to satisfy
his thirst.
Curiosity:
- What
aggravated asthma of Kishan Lal?
- How the problem was
solved?
Kishan Lal's
asthmatic problem was solved as he
stopped taking soft drinks. Colouring agents of the soft drinks
and sqaush aggravated Kishan Lal's asthma.
Expert Comments:
It is not an isolated problem of Kishan Lal but
inspite of modernisation and advancement in therapy,
the prevalence of asthma is increasing. More people
are dying out of a fatal asthma in recent years than
preceding decades. Asthma specialists explained this
asthma dilemma and held responsible the modern food as
an important trigger factor. In an interesting study
carried out in South Africa it was observed that
asthma prevalence was quite low in rural Africa in
comparison to urban areas. Rural African take natural
food and live a traditional life style. In contrast in
Johannesburg people were fond of taking preserved and
processed foods, lived a modern life style and exposed
to higher magnitude of environmental pollution.
Subsequently in many parts of world these
observations were proved. With inventions and
innovations we are trying to make our life comfortable
but we have to pay price for it. It appears that
higher asthma prevalence is also a penalty of
modernisation and modern food.

After
how much time food causes problem?
Symptoms
of allergy to food taken may appear either soon after, or
several hours later; in the latter case, it becomes difficult
to identify the offending article of diet. Food allergy is more common in infants and children. As a
child grows older, he is exposed to many more environmental
factors. That is why adults are frequently sensitive not only
to foods but also to pollens, dusts, molds, etc. as well.
Should
an allergic person avoid Milk?
Cow's
milk is not an uncommon allergen in infancy, probably because
it is the major foreign protein food during the early months
of life. It has been estimated that milk allergy occurs
approximately in 0.5 percent of children. Milk allergy manifest itself by a variety of symptoms. The
commonest symptom is that of the infant feeling unwell when he
is put on a milk other than that of his mother, and a need is
felt to change from one milk to another. Other symptoms
pertain to alimentary tract and respiratory tract. There may
also be eczema or urticaria. Whenever possible the infant should breast-fed. This reduce
the chances of the development of allergic symptoms. Giving
cow's milk should be delayed up to the age of 9 months to a
year in a potentially allergic child.
Many a time, the diagnosis is missed because allergy to milk
is not suspected. The best way to make a diagnosis is by
eliminating milk from the diet and then rechallenging the
process to administer antigen to evoke an immunological
response. Milk must be eliminated from the diet for at least
two weeks. If during this period there is marked improvement
in the health of child, or when reintroduction of milk brings
back the symptoms, the diagnosis is established. After the diagnosis is clinched, the offending milk must be
removed from the diet. Milk prepared from soya beans is a good
substitute. If it is not locally available, it can be prepared
in the house. The soya bean milk should be given up to a year or so and then
a re-trial could be made with the offending milk. If the child
does not tolerate it, soya bean milk may continue to be given;
if few symptoms appear then the cow's milk may gradually be
added in the diet. Soya bean milk is as nutritious as mother's milk; however,
this needs to be supplemented with vitamins and minerals.
Wheat Allergy
Next
to milk, wheat is the commonest source of food allergy. This
is detected when the infants for the first time is given
cereal, and develops a rash, abdominal colic or diarrhoea. An older child or an adult may have asthma or a patch of
eczema which is found to be improved by not taking wheat in
any form. The treatment is avoidance of wheat in all its
different preparations.
Egg
Allergy
White
of an egg is pure albumin ( a protein) and being a foreign
protein, it is potent source of allergy. Taking of a raw egg
by a person allergic to it, can cause a most severe reaction,
including collapse and shock. Even the smell of egg's contents
can cause allergic reactions in highly sensitive people. |

Fish Allergy
Fish
and other sea foods are amongst the most potent allergic
agents. There is a cross reaction among various sea foods. Those who
have reaction to fish should avoid other sea foods as well.
Here again, even the smell of fish can produce urticaria in
some people who are strongly allergic to it.
Other Food Items
Most of the nuts, e.g. almond, walnut can cause allergic
reactions. Persons who are allergic to them have to avoid them
and also preparations like cakes and cookies containing nuts. Other food to which people are commonly allergic are: apples,
bananas, cashew nuts, oranges, lemons, grapefruit, mushrooms,
watermelons, cucumbers, grapes, gur (not cane-sugar), mustard,
radish, onion, garlic, chocolates and soft drinks.
Diagnosis
A
thorough history of patient is of the greatest help in
diagnosis. Other procedures that help are elimination
diets. Skin test in food allergy are not of much help.
Elimination Diets
The patient is
instructed to note on a piece of paper all the food he takes
and the symptoms, if any, they produce. By studying these
daily reports for two or three weeks, one can often detect the
onset of symptoms after a new food or after the repeated use
of certain food. The elimination of these foods from the diet
one by one, should result in disappearance of symptoms, and
the intentional ingestion should be followed by symptoms.
The difficulty
arises when the offending food is either milk, egg or wheat,
each of which is a common ingredient of many other foods so
that its presence is not generally suspected by the patient.
It is incumbent on the patient to study and know the
ingredients of the food he consumes, so that, he may be better
able to avoid any or all of these foods in his diet.
Skin test
Many
times, there is no correlation between the skin tests and the
clinical trials with a food. This is because the tests are
made with foods extracted in the raw state but eaten in the
cooked state, and it has been shown that the cooking of some
foods may denature them to the extent by the individuals
sensitive to them. While food trials are being conducted, it
is essential that all other treatment remains unchanged. The best method of treating food allergies is to find out the
incriminating food and to avoid it. While eliminating one
food, it is necessary that a non-allergenic substitute of
equivalent food value be added to maintain nutrition and avoid
monotony. Desensitization to foods is seldom resorted to. Where the offender cannot be identified, one can only depend
on symptomatic therapy. Along with this the patient is
advised to take, as far as possible, cooked foods which
being denatured are less allergenic. Diet
for Asthmatic Patients
Proper
diet taken at regular times is very important for a patient of
asthma. This can not only prevent an attack from occurring but
also can lessen the severity of the attack that a patient is
having.
The diet should be light, nutritious and well balanced. It
should not contain such things which do not go well with the
patient. Experience shows that for north Indians, rice taken
at night causes discomfort in breathing. This may be because
of the fact that to satisfy hunger with rice, one has to take
a large quantity of it, so that the stomach is blotted and
this put hindrance in the way of the full and proper working
of the diaphragm in respiration. Other things that patients
generally complain of not being comfortable are taking
bananas; among the vegetables ladyfingers and among the
lentil, urad and its dal. So these may be
avoided. Spices should be avoided in diet. With some people,
cold drinks and sour preparations like chuttnies and
pickle cause coughing; so these may be avoided.
Asthmatic patient should not take full-stomach meal. Fullness of
stomach after meals in particular at night causes discomfort
in breathing or even precipitates breathlessness. One should
take less quantity at a time, though he may take for more
numbers of times. Fried things should be avoided as far as
possible.
No sweet dish would be taken at night. A patient of asthma
would notice that he feels more comfortable during the night
if at the time of sleep his stomach is not heavy. So dinner
should be taken at least two hours before going to bed. Asthma patient should take a variety of vegetables, cheese, eggs
and chicken , soups and fruits. But all of these should be
taken in moderation. There should be adequate variety in the
diet of the patient so that he does not feel bored with it.
It is preferable that an asthma patient avoids taking food
outside his house, particularly the party dishes; they are all
heavy with fat and frying. However, in order to avoid
attention towards oneself, one may, if it is possible, take
lighter meal than usual with one or two innocuous looking
dishes.
Diet
for children
Children
who develop asthma do not pick up weight or even lose weight.
So special attention has to be given to their diet. A child requires more calories per kilogram of body weight
than an adult, because 1) his basal metabolic rate is higher,
2) he indulges in greater physical activity, and 3) more
calories are needed by him for the growth of his body. An
asthmatic child has even greater metabolic rate, because of
hurried breathing.
So the child needs more carbohydrates and more proteins. If
the diet is otherwise well balanced, no extra addition of
minerals and vitamins are necessary. A liberal amount of water
should be given to child asthmatics, because they lose a lot
of it in respiration. Meal hours should be regular, and as far
as possible, nibbling in between meals should not be allowed.
A child may be told what he should not take as for example,
pickles,
chuttnies etc. he should not, however, be unduly forced to
take a particular diet which he does not like. On the other hand,
his likes and dislikes should be anticipated and variety
introduced in the diet which is well balanced. If the child is otherwise
healthy-eats well, plays well,
sleeps well- but does not take adequate food according to the
parents standard, then it is parent and not the child who
needs to be instructed. While some children like to live disciplined lives, other's
don't.There are some children to whom milk-taking day after
day is monotonous; for such children variety in serving milk
preparation should be found.
Taking more sweets while keeping the calories adequate in
diet, makes it deficient in proteins.
School-going children often hurry through their breakfast
because they get up late, and then hurry through their lunch,
because they have to play. And then take a heavy dinner. All
this is wrong. A full stomach while going to bed is more
liable to lead to an attack of asthma during the night.
Diet
for aged people
In
old age calorie requirement is reduced because of
lessened activity and lowered metabolic rate. If such person
is suffering from some other disease like arthritis or angina,
the activity is further reduced. Aged asthmatic persons on
corticosteroids occasionally or regularly, are on increased
risk of developing osteoporosis (thinning of bones). Both old
age and effect of steroid drug causes thinning of bone by
inducing deficiency of calcium, protein, vitamins, minerals
and hormones. It can cause fracture of bones even without
significant trauma. It is not uncommon to find in an x-ray of
the chest that one or more of the vertebrae are broken and
collapsed. The whole body of an older asthmatic is shrunken-
the height, the muscles, the girth of the abdomen.
In such patients special diet is important. Calorie content of
the diet should be increased. When the patient feels better,
he must be asked to take more diet so as to regain weight.
Intake of proteins and vitamins such as milk, cheese, pulses,
soyabean should be encouraged. If he is non-vegetarian, then
he can add chicken or its soup, meat, egg and fish, provided
he is not allergic to any of these. For the sake of variety in
diet, custard, pudding can be taken, particularly so if
mastication is the problem because of loss of teeth. Intake of
fat should be limited. Anyone of the multivitamin tablets with
mineral should be added to the daily diet.
Injections of one of the anabolic steroids are helpful in
older people to regain protein retention; Some increase in
weight and a general feeling of being energised is observed.
